Why LDL Cholesterol Rises During Perimenopause and Menopause—and What Can Help Lower It

Have you been eating well, exercising, and maintaining a healthy weight—only to discover that your LDL cholesterol is suddenly higher?


You are not alone. An unexpected rise in LDL cholesterol during perimenopause and menopause is extremely common, and it can be frustrating when you feel like you are doing everything right.


At Dragonfly Menopause Care, we frequently see women in their 40s and 50s who develop higher LDL cholesterol during the menopause transition. Often, it is not simply a lifestyle problem. Genetics, changing estrogen levels, inflammation, insulin resistance, thyroid function, and weight changes can all play a role.


The good news: there are often several ways to improve your cholesterol profile, depending on your personal risk factors and health goals.


Why does LDL cholesterol rise during perimenopause and menopause?


1. Falling estrogen levels can increase LDL cholesterol


Estrogen helps influence how the liver clears LDL cholesterol from the bloodstream. During perimenopause, estrogen levels can become highly variable. After menopause, estrogen levels decline further.


As a result, many women see LDL cholesterol rise during perimenopause or menopause—even when their diet and exercise routine have not changed.


This is one reason cholesterol may look very different in your late 40s or 50s than it did in your 30s or early 40s.


2. Genetics often play a major role


For many women, high LDL cholesterol is largely genetic. If you have had elevated cholesterol since young adulthood, or if your family has a history of high cholesterol, premature heart disease, stroke, or familial hypercholesterolemia, lifestyle changes may improve LDL but may not completely normalize it.


A rising LDL is not a personal failure. It is often biology.


3. Weight gain and insulin resistance can worsen cholesterol


Menopause is associated with changes in body composition, including a greater tendency toward abdominal or visceral fat. This can worsen insulin resistance and lead to unfavorable changes in LDL cholesterol, triglycerides, ApoB, and blood sugar.


Not every woman with high LDL needs to lose weight. But when weight gain or insulin resistance is contributing, modest and sustainable weight loss can improve cholesterol, inflammation, blood sugar, and cardiovascular health.


4. Dietary fat quality matters more than simply eating “low fat”


A healthy cholesterol plan does not require a low-fat diet. Healthy fats are important for hormones, satisfaction, and overall wellbeing.


Instead, focus on reducing unhealthy fats and choosing better fat sources more often. Frequent intake of butter, cream, cheese, processed meats, fatty red meat, coconut oil, fried foods, and ultra-processed foods can increase LDL cholesterol in some people—especially those who are genetically sensitive to saturated fat.


Try replacing some of these foods with:

  • Olive oil

  • Avocado

  • Nuts and seeds

  • Fatty fish

  • Beans and lentils

  • Fiber-rich whole foods


5. Inflammation can affect LDL and HDL cholesterol


Chronic inflammation can contribute to increases in LDL cholesterol and undesirable changes in HDL cholesterol. A higher HDL number is not always automatically protective; we need to consider the whole metabolic and inflammatory picture.


Poor sleep, chronic stress, autoimmune or inflammatory conditions, smoking, insulin resistance, excess visceral fat, and an ultra-processed diet can all contribute to inflammation and cardiovascular risk.


6. Thyroid changes can raise LDL cholesterol


Hypothyroidism is another common cause of rising LDL cholesterol. Thyroid function is important to consider when cholesterol rises unexpectedly, especially if you also notice fatigue, constipation, dry skin, feeling cold, hair changes, or weight gain.


How to lower LDL cholesterol without statins


The right treatment depends on your LDL level, family history, metabolic health, ApoB, lipoprotein(a), inflammation, and overall cardiovascular risk. Some women need prescription cholesterol medication. Others may benefit from lifestyle changes, HRT, weight loss, and carefully selected supplements.


At Dragonfly Menopause Care, we look at the whole picture—not just one number on a lipid panel.


Menopause hormone therapy (HRT)


For women with menopausal symptoms who are appropriate candidates for hormone therapy, HRT may have a favorable effect on LDL cholesterol and the overall lipid profile.


HRT is not prescribed solely as a cholesterol medication, but it can be an important part of a comprehensive menopause and cardiometabolic plan. In my practice, I have seen LDL cholesterol decrease by approximately 30 points with HRT, weight loss when needed, or a carefully selected cholesterol-support blend.


Weight loss, when appropriate


If insulin resistance or excess body weight is contributing to high LDL, gradual weight loss can improve LDL cholesterol, triglycerides, blood sugar, blood pressure, and inflammation.


The goal is not perfection. A sustainable plan that includes adequate protein, strength training, fiber, sleep support, and realistic nutrition changes is far more valuable than restrictive dieting.


Soluble fiber and a cholesterol-supportive diet


Soluble fiber can help support healthy cholesterol levels. Great sources include:

  • Oats

  • Beans and lentils

  • Chia seeds

  • Ground flaxseed

  • Apples and berries

  • Psyllium


Regular exercise also matters. A combination of resistance training, walking, and aerobic exercise supports insulin sensitivity, body composition, inflammation, and cardiovascular health.


Red yeast rice extract


Red yeast rice extract can be an effective non-prescription option for lowering LDL cholesterol. It contains monacolin K, a compound chemically identical to lovastatin.

I have a favorite red yeast rice extract in a blend that I often recommend for my patients, although there are many options available.


Red yeast rice should still be treated with caution. It can cause muscle aches much like statins, can interact with medications, and product quality and potency vary greatly. It can also lower CoQ10, so I recommend taking CoQ10 alongside it.


Always discuss red yeast rice with your clinician before starting it, especially if you take prescription medications, have liver disease, have experienced statin-related muscle symptoms, or are pregnant or breastfeeding.


Berberine


Berberine is one of my favorite supplements because it offers benefits beyond cholesterol support. It can be helpful for insulin sensitivity and glucose metabolism, and it may support a healthier microbiome.


For women with elevated LDL cholesterol along with insulin resistance, elevated blood sugar, elevated triglycerides, or weight concerns, berberine can be a particularly useful tool. It can interact with medications and may cause gastrointestinal side effects for some people, so it should be used thoughtfully.


Alpha tocotrienols


Alpha tocotrienols are members of the vitamin E family that may support healthy cholesterol metabolism. Their LDL-lowering effect is generally more modest than a statin or an effective red yeast rice product, but they can be a helpful part of a broader cholesterol plan.


Pomegranate juice


Pomegranate juice is rich in polyphenols and can be an easy addition to an anti-inflammatory, heart-healthy routine. I often recommend ¼ cup of unsweetened pomegranate juice daily.


Should you get an advanced lipid profile?


A standard cholesterol panel is helpful, but it does not tell us everything.


At Dragonfly, I often recommend an advanced lipid profile, which is a simple blood draw. This can help us better understand your LDL particles, including how many are smaller, denser LDL particles that may be more concerning from a cardiovascular standpoint.


Depending on your history, we may also look at:

  • ApoB

  • Lipoprotein(a), or Lp(a)

  • Triglycerides and HDL cholesterol

  • Blood sugar and insulin-resistance markers

  • Thyroid function

  • Family history of heart disease or stroke


Should you get a coronary artery calcium score?


If we are trying to determine how aggressively to lower your LDL cholesterol, a coronary artery calcium score can be very helpful.


This is a quick CT scan that looks for calcified plaque in the coronary arteries. A calcium score can provide more individualized information about cardiovascular risk and may help guide the decision about lifestyle, supplements, HRT, or prescription cholesterol medication.


The bottom line: High LDL in menopause is common—and treatable


An elevated LDL cholesterol level deserves attention, but it does not deserve shame. The menopause transition often reveals a genetic tendency toward higher cholesterol, even in women with healthy habits.


Sometimes lifestyle changes are enough. Sometimes HRT, weight loss, supplements, prescription medication, or a combination is appropriate.
