Genitourinary Syndrome of Menopause: Why Vaginal Estrogen Is a Game-Changer

If you’ve been in my clinic recently, you’ve probably heard me say this: vaginal estrogen is one of the most underrated, safest, and most effective treatments I offer. And nothing highlights this more than the women I saw just yesterdaytwo separate patients whose years of recurrent UTIs completely disappeared once they started vaginal estrogen.

This happens all the time in my practice, and honestly… it still blows me away.

But before we dive in, let’s talk about what’s really going on.

What Exactly Is Genitourinary Syndrome of Menopause (GSM)?

GSM is the newer, more accurate term for what many know as “vaginal atrophy.” It includes changes to the:

  • vagina

  • vulva

  • bladder

  • urethra

  • pelvic floor


As estrogen levels fall in perimenopause and menopause, these tissues become thinner, dryer, more fragile, less elastic, and less resistant to bacteria. This leads to:

  • UTIs (sometimes monthly, sometimes constantly)

  • burning

  • urgency

  • pain with intercourse

  • “I think I have another UTI” symptoms (but the urine test is normal)

  • pelvic pressure

  • vaginal dryness or tearing

  • recurrent BV or yeast


And here’s the important part: GSM does not get better on its own. It is progressive—until we replace the estrogen that those tissues desperately need.



Vaginal Estrogen: Small Dose, Big Impact

Vaginal estrogen is truly micro-dose estrogen. It works locally, stays local, and restores the tissues in a way nothing else can.


What improves when we use it?

  • UTIs dramatically decrease (or stop!)

  • Bladder urgency can calm down

  • Vaginal moisture returns

  • The pH normalizes

  • Sex becomes comfortable again

  • Pelvic floor muscles respond better

  • The urethra regains its healthy tone

  • The microbiome shifts in a protective direction


For many women, it feels like “getting your bladder and vagina back.”

It’s so safe that—for years—I prescribed it from the ER for women with recurrent UTIs. Truly… how could I not?



The Black Box Warning Has Been Removed — Why This Matters

For two decades, vaginal estrogen carried a frightening black-box warning borrowed from oral/systemic estrogen, even though:

  • vaginal estrogen does not raise estrogen levels into menopausal ranges

  • it does not increase the risk of breast cancer

  • it does not increase clot risk

  • it does not increase stroke or heart disease risk

This warning dramatically limited how comfortable many physicians felt prescribing it.


As of 2025, the FDA has officially removed this warning.

This means:

  • The label now reflects what research has shown for years: it’s safe.

  • Physicians who were previously hesitant can now prescribe with confidence.

  • Women should have far fewer barriers to receiving this therapy.

  • We can finally move toward treating GSM as the medical condition it is—not something women must “just deal with.”


This is a huge step forward for women’s health.

  • Estrogen improves arterial flexibility

  • It decreases plaque formation

  • It lowers cholesterol

  • It supports healthier blood pressure

  • It improves nitric oxide and vascular function


This isn’t why we prescribe it—but it’s certainly a wonderful side benefit.



Bottom Line: GSM Is Treatable, and You Deserve Relief

You do not need to live with:

  • recurrent UTIs

  • bladder urgency

  • burning

  • dryness

  • painful intimacy

  • feeling like “something is off down there”


GSM is medical.It is common.It is treatable.


And vaginal estrogen is one of the best, safest tools we have.
